1. Download and print the applicable patient intake forms:
   • Non-Medicare Patients
   • Medicare Patients
   • Consent to Treat a Minor
   • Dry Needling Consent & Information Form
2. Then, download and print the forms that pertain to the condition you are having treated:
   • DASH - Arm, Elbow or Hand Problem
   • Fall - Balance Issues
   • Lysholm - Knee Problem
   • Neck - Neck Pain
   • OSWESTRY - Back Pain
   • Shoulder - Shoulder Pain
   • Lower Extremity - Hip or Ankle Pain
   • Patient Specific - Multiple Issues Not Addressed by One Form
3. Bring the completed forms with you when you come in for your first visit.
